A dashboard page's header: core's (phoxtail_core/page_header.html), its icon
and name those of the page's nav entry, so the page is named as the menu
names it. A page under an entry (a project under Projects) passes its own
page_title; a page with no entry (Profile) passes page_icon too. A page's own
header extends this one for its controls.
