3-D local-maxima (peak) detection.
A voxel is a peak if it equals the maximum over a cubic neighbourhood of
half-width `min_distance (side 2*min_distance + 1`) and that
neighbourhood is not flat (its max strictly exceeds its min, so a constant
region yields no peaks). Pass *threshold* to additionally require
`vol >= threshold` (drop weak peaks in noise).
Returns an `(N, 3) int array of (z, y, x)` peak coordinates. Uses only
`scipy.ndimage.maximum_filter / minimum_filter` (no skimage).
